Job duties :

• Dispenses medications for outpatient.
• Recommends formula alternatives when appropriate.
• Provides drug information and education to outpatient 
• Reviews and asseses medication orders to identify potential and actual drug related problems.
• Report medicaions errors to senior outpatient if present to facilitate care
• Prevent and resolve potential and actual drug related problems
• Participates in approved collaborative practice agreements if necessary

Job qualifications:

• Bachelor degree in pharamcy 
• 2 years of experience, hospital pharmacy expereince is preferred  
• Male phramacist is a preference
• English language, proficency is required
